This survey is intended to gather information regarding only the bank’s Personal DDA Accounts.  Do NOT include in your survey responses data regarding Business DDA accounts or MMDAs.  When querying your bank’s Personal DDA account data, only query accounts with an “open” status code (data for “dormant” or “inactive” accounts should be excluded) – this is VERY IMPORTANT.  

The data reported for transaction volumes (Lines 5, 11, 12, and 13), and for fee income (Lines 6, 7, 19, 20, 21 and 23.b) are for the six-month period from January 1, 2017, through June 30, 2017.  The data reported for the dollars and number of accounts (Lines 1, 2 ,3, 4, 8, 9, 10, and 14) are as of June 30, 2017.  Only the dollar amount reported on Line 1 should be reported in thousands ($000) – all other numbers should be reported in their actual amounts.
